UTILITY ASSISTANCE APPLICANTS
PLEASE NOTE THE FOLLOWING REQUIREMENTS FOR UTILITY
ASSISTANCE APPLICANTS:

         
1) YOUR UTILITY MUST BE DISCONNECTED. THIS MEANS YOU
    DON’T   HAVE ANY SERVICE! (WE DO NOT ASSIST WITH "FINAL
    NOTICES PRIOR TO DISCONNECTION", YOU MUST BE
    DISCONNECTED)

2) YOU MUST HAVE A VALID EMAIL ADDRESS IN ORDER TO
    RECEIVE YOUR CONFIRMATION.

3) IF YOU DON’T HAVE A VALID EMAIL, WE WILL NOT CONTACT
   YOU, YOUR APPLICATION WILL NOT BE ACCEPTED, AND THE
   APPLICATION IS VOIDED.

  
THERE ARE NO EXCEPTIONS TO THE ABOVE RULES.

4)  YOUR UTILITY BILL MUST BE A MINIMUM OF $500.00 TO QUALIFY
    FOR THE PROGRAM.

5)  IF YOUR SERVICE IS DISCONNECTED, AND YOU HAVE A VALID
    EMAIL ADDRESS, PLEASE PROCEED TO COMPLETE YOUR
    APPLICATION FOR ASSISTANCE.

6) YOU MUST HAVE A CONFIRMATION LETTER IN ORDER TO  
   SCHEDULE AN APPOINTMENT.
